How dangerous is a Cardiac catheterization and possible stent placement on someone who takes coumadin?
My mom had a nuclear stress test done and they found 2 possible blockages. They are doing a cath this week and placing stents if they can. She takes coumadin because she has had her mitral valve replace, but is going off it today and they will use heparin in the hosp. She is allergic to heparin, but they said they have to use it. They found all this because 2 weeks ago her heart started beating 150 beats a minute and she was taken to emergency. They have controlled that with medication, but the stress test showed her heart wasn’t getting enough oxygen during exercise, so they are doing this cath. I am worried because of the blood thinner and wonder if anyone can let me know how much more dangerouse this causes the procedure to be.
I assume that they are admitting her to the hospital a few days prior to this to stop her coumadin and start her on a heparin drip? That is normal protocol. Coumadin stays in your system for several days. So, if you stop taking your coumadin on day 1, then your blood will still be “thin” through around days 3-5. Heparin on the other hand only stays in your system for a short time. They need to keep her on the heparin until they do the heart cath and then they can keep her on it until they get her coumadin and INR back where it needs to be. She should not be in any more danger utilizing this method than she would be otherwise. When you say she is “allergic” to heparin, what kind of reaction does she have to it?

12 Lead Ecg Interpretation In Acute Coronary Syndrome With Case Studies From The Cardiac Catheterization Lab $84.15 This 304 page full-color textbook is essentially two volumes in one. The first 92 pages present the basic principles of 12 lead ECG interpretation. The remainder of the book focuses on the evaluation of Acute Coronary Syndrome (ACS) patients. 13 ECG patterns associated with ACS are presented, including the most subtle ECG changes most often missed by clinicians and the ECG machine’s automated interpretation software. The curriculum provides an in-depth, balanced approach to patient assessment, combining advanced ECG interpretation skills with patient history, risk factor and cardiac marker evaluation. Correlation of ECG leads with the coronary arterial distributions which commonly supply each region of the heart are reinforced by use of 24 case studies of ST Segment Elevation Myocaridal Infarction (STEMI), Non ST Segment Elevation Myocardial Infarction (NSTEMI), Unstable Angina, and Brugada Syndrome. STEMI Case Studies emphasize the ECG identification of the infarct related artery, and complications to be expected based on failure of cardiac structures commonly supplied by the obstructed arterial distribution. Case studies begin with patients’ initial assessment data obtained in the emergency department and continue into the cardiac catheterization lab where coronary artery angiography illustrates the location of arterial obstruction. Key learning objectives for each type of Myocardial Infarction (MI) are highlighted. 453 full color images, 135 review questions, and 114 ECGs reinforce the learning process. Target audience is all medical professionals whom are already competent in single-lead rhythm strip evaluation, and desire to become proficient in the 12 lead ECG evaluation of Acute Coronary Syndrome patients. This book was written and reviewed by veteran cardiac cath lab interventionalists. View this book’s Table of Contents on the publisher’s website: www.TriGenPress.com |

With increasing numbers of procedures such as cardiac catheterization and imaging taking place outside of the main OR, anesthesia providers as well as non-anesthesia members of the patient care team will find this book critical to their understanding of the principles of anesthesia care in unique settings which may have limited physical resources. Topics include patient monitoring techniques, pre-procedure evaluation and post-procedure care, and procedural sedation performed by non-anesthesia providers. The authors address problems of anesthesia that have unique answers in OOR settings, such as patient transport and cardiac arrest, and discuss technological progress and considerations for the future. The text also covers surgical procedures and anesthetic considerations by procedure location, such as radiology, infertility clinics, field and military environments, and pediatric settings, among many others Select guidelines from the American Society of Anesthesiologists (ASA) are provided as well. Edited by the senior faculty from Harvard Medical School and with contributions from other academic institutions, Anesthesia Outside of the Operating Room provides a unique and convenient compendium of expertise and experience. |

Anesthesia for Congenital Heart Disease $224.95 Anesthesia for Congenital Heart Disease provides a state of the art summary of the many changes occurring in this rapidly evolving field over the last decade and will prove to be invaluable to pediatric anesthesiologists and cardiac anesthesiologists alike All the chapters are written by leaders in the field of congenital cardiac anesthesia, who perform large numbers of cases clinically, and contribute important knowledge to the research literature, both clinical and basic science. A wide range of contributors from all of the major congenital heart surgery programs in North America give a broad range of perspectives not seen in previous texts in this field. Anesthesia for Congenital Heart Disease is the most comprehensive, detailed and specific, from the perspective of multiple institutions in its field. There are new chapters on subjects that have never before been addressed in a textbook of pediatric cardiac anesthesia including cardiac catheterization laboratory anesthesia, computers and technology, neurologic monitoring, bleeding and coagulation, approach to the teenager and adult, approach to the premature newborn, the inflammatory response, regional anesthesia and pain management, airway and ventilatory management, dysrhythmia management, non-cardiac surgery and cardiac magnetic resonance imaging; and research, teaching and administration. Emphasis on new and emerging developments in anesthetic drugs and techniques occurs in each chapter. |

Balloon Septostomy $45 High Quality Content by WIKIPEDIA articles! Balloon septostomy is the widening of a foramen ovale, patent foramen ovale, or atrial septal defect via cardiac catheterization using a balloon catheter. This procedure allows a greater amount of oxygenated blood to enter the systemic circulation in some cases of cyanotic congenital heart defect. After the catheter is inserted, the deflated balloon catheter is passed from the right atrium through the foramen ovale, PFO or ASD into the left atrium, it is then inflated and pulled back through to the right atrium, thereby enlarging the opening and allowing greater amounts of blood to pass through it. |
